Acknowledgment and Assumption of Risk
I, the undersigned participant, acknowledge that participation in art classes at MOA Art Studios, MOA Multimedia Arts and Architecture Inc. involves the use of materials and equipment that may pose risks, including but not limited to, exposure to paint, solvents, sharp tools, or other potentially hazardous materials. I understand that while MOA Art Studios takes reasonable precautions to ensure a safe environment, accidents, injuries, or damages may occur. I voluntarily assume all risks associated with my participation in these classes.

Release of Liability
I hereby release, waive, and discharge MOA Art Studios, its owners, instructors, employees, and affiliates from any and all claims, liabilities, demands, actions, or causes of action arising out of any loss, damage, injury, or harm that may occur during or as a result of my participation in the art classes, except in cases of gross negligence or willful misconduct.

Health and Safety
I confirm that I am in good health and have no medical conditions that would prevent me from safely participating in the art classes. I agree to follow all safety guidelines and instructions provided by MOA Art Studios and its instructors. If I have any allergies or medical conditions that may require special consideration, I will inform the studio prior to my participation.

Personal Belongings
I understand that MOA Art Studios is not responsible for the loss, theft, or damage of any personal belongings brought to the studio.

Media Release (Optional)
I grant MOA Art Studios permission to take photographs and videos of me during the art classes for promotional, advertising, and social media purposes. I understand that I will not receive compensation for the use of these images.

Alcohol, Drug, and Conduct Policy
If consuming alcohol during a class, I confirm that I am of legal drinking age in Ontario (19+). I understand that I must drink responsibly and that MOA Art Studios reserves the right to remove any participant who behaves inappropriately or disrupts the class. Excessive intoxication, disorderly conduct, or any behavior that puts others at risk will result in immediate dismissal from the class without a refund.

The possession, use, or distribution of illegal drugs or controlled substances is strictly prohibited on the premises of MOA Art Studios. Any participant found violating this policy will be immediately removed from the premises and may be subject to legal action.

By signing below, I acknowledge that I have read and understood this waiver and agree to its terms voluntarily.
